Vault provides unlimited space for notes, allowing the clinician to record patient-specific risks and any additional points discussed with the patient. The patient has the opportunity to review the notes and agree that they are an accurate summary of the conversation.
Once the patient has verified their information and is fully informed, they will sign the electronic document with a stylus. The clinician will also sign and both signatures will be included in the final PDF version of the consent form.
